  - Add A New Asset Worksheet (PDF - 12 KB) - This form should be used when equipment has been newly purchased or donated. It is not to be used when a transfer is made.

  - Disposal Form (PDF - 13 KB) - This form should be signed by your principal, director, or technician prior to sending to the County Office. Do not dispose of any equipment until the School Board approves the disposal request.

  - Donated Item (PDF - 8 KB) - Any item that meets the criteria for an asset that is donated to Pontotoc County School District by individuals, booster clubs, or activity funds must be School Board approved.
  - Lost or Stolen Affidavit (DOC - 23 KB)
  - Lost or Stolen Affidavit (PDF - 21 KB) - The person accountable for the lost or stolen equipment must have their signature notarized on the affidavit. Each campus has a secretary that is a notary. Then forward the affidavit to the County Office for School Board approval to remove from inventory.

  - Transfer Equipment (PDF - 15 KB) - After equipment has been transferred from one room to another, the person taking responsibility will need to sign the form before sending the form to the County Office.

- Medical or Dental Prior Approval Form (DOC - 28 KB)
- Medical or Dental Prior Approval Form (PDF - 19 KB) - According to Mississippi Code Section 37-13-91(e), all medical and dental appointments must be preapproved for the absence to be an excused absence. The only exception is in the case of an emergency.

- Picture Release Form (PDF - 19 KB) - This form gives permission for a picture of your child or the work of your child to be displayed. At no point will student grades, individually or by group, be displayed on the web pages. See the form for details.
